In a sense, both. Gluttony and I have been researching about these phenomenons and went out to seek answers ourselves. Pride had their own specific answer they wanted to find, and Envy felt like coming along. We were purposefully looking for a way, but we were still caught off-guard anyhow.
We started in the same location, but entering the door split us apart. From what I've vaguely heard, we all experienced something similar but with enough differences it felt very...targeted.
It was the same for me, but I'm guessing it's the same for everyone that went. I haven't cross referenced stories with others. We kind of have a cagey bunch.

We've compared the fundamentals of the trips as a whole like being taken away somewhere and finding your guys' items, but from what I remember we mulled more about the essence—a trial or something. No one said specifics aside from myself when I clarified what was expected of my group when we wanted to leave. Wasn't hard to tell everyone had a hard time, though.
I personally think location is pretty important... Childe told me his was "Hexagon Island" and the location would change, but it's related to someone with him? When I got magicked away, it was a grim hotel with a dark hallway and the location did not change aside from this door on my floor. Might have been the same for the others. Anyway, the hotel wasn't familiar to me nor anyone I was with.

Secondly, there are always people, objects and tasks that are familiar, usually those designed to test your resolve without any sort of relief. And lastly, there's an underlying theme that ties into one of our namesakes, regardless of who is accounted for.
I had initially thought it was a test of your will, to push you and see how far you were willing to go in order to obtain your desires. While that's likely true, that again is where the difference lies between us and yourselves. Ours wasn't a test.

Logically speaking, you're correct. There's a failsafe with you avatars where you will survive even when losing a whole head, so if anything it would be the most efficient method. You are all clearly stronger than us, too. Progress would be steady, or at least steadier than nothing. The issue would be being trapped wherever it is forever, if that's possible.
